Pandemic is already wickedly hard, but the addition of a bioterrorist makes it nearly impossible!
I’m never going to win again!
You need the base game to play.
There are 3 different challenges/ways to play with this expansion and each one is fun and uniquely challenging. While you can play with 2 players, it’s a lot easier of a game playing with more as you can use more roles in your favor.
Theres also another expansion you can play that requires the base game and this expansion.
Definitely recommend if you’re a fan of the game!

You need the base game to play.
There are 3 different challenges/ways to play with this expansion and each one is fun and uniquely challenging. While you can play with 2 players, it’s a lot easier of a game playing with more as you can use more roles in your favor.
Theres also another expansion you can play that requires the base game and this expansion.
Definitely recommend if you’re a fan of the game!
If you haven’t played Pandemic before, I definitely recommend starting with the base game.
That said, it meshes perfectly with the 2nd edition (when that came in later), and adds a lot more complexity that I and my fellow players appreciated.
The addition of the BT adds a "scotland yard" twist to the game, though the players tend to have a difficult time adjusting to this - they focus on capturing the BT the first few times, when in reality the BT simply bides his time in capture and escapes... his capabilities are hardly diminished simply because he is captured. The players really need to focus on cures more than the BT, and it's hard to make that decision (particularly when the BT is thumbing his nose at the other players) :).
With the "Virulent Infection" piece of the expansion, epidemics can now have side-effects that change the way you deal with infections. Some of these are immediate effects that are resolved on the spot, while others have on-going effects that continue until the disease is cured. This added complexity meshes well with the base game without necessarily increasing the difficulty by much - yes, it's more complex, but it's not necessarily more difficult.
The "Mutation" expansion, however, does increase the difficulty quite a bit. You only get 12 mutation cubes, compared to the 24 you get for the other diseases. This means when you run out of mutation cubes in the infection phase, it's game over... so mutations need to be dealt with quickly. I've had quite a few games end early simply because the players forgot about the 12-cube limit until it was too late. On the other hand, I enjoy the game more with the mutation expansion in play, so despite the added difficulty, it's still a lot of fun to play.
Set-up: The set-up for the On the Brink expansion takes roughly the same amount of time as the base Pandemic game. Just a few minutes to create the player & infection decks, then infecting cities around the world.
Difficulty: Having only played the Virulent Strain and Mutation variants, the learning curve was very similar to learning the base game. These variants require the team to remember new conditions that are in play, and are fairly straightforward. The Bio-Terrorist seems to have new rules on movement and actions, and would require the person playing the Bio-Terrorist to understand the rules well since their movements are hidden.
Replayability: Each of these variants seem to add more conditions/events in the gameplay, and I believe you are able to include multiple/all variants for added variability. On the Brink also adds more event cards and role cards.
Final Thoughts: 9/10
The variants that I have played add some refreshing changes to a great game, although the magnitude of the changes greatly depends on the randomness of the draw. I also like the petri dishes that came with the game to hold the disease cubes. Nice touch. With the new role cards and events, I will probably always add elements of the expansion to the game, even if I don't play the different variants.

The new roles are key as well and really unique. Their skills definitely help with the mutation virus, such as being able to pick up a discarded card, having 8 cards in your hand or being able to peek at the top infection cards before your turn.
There are also new event cards. Things like being able to swap out a character for a different one, also makes the game entirely different.
I only wish I bought this expansion earlier. It is amazing and if you are a pandemic fan, this will blow your mind for a next level pandemic experience.

Über Pandemie bzw. Pandemic ist viel Positives geschrieben worden, dem kann ich mich uneingeschränkt anschließen. Nur eines möchte ich besonders herausheben: Ich fand "kooperative" Spiele immer ein bisschen lahm, weil leicht die Spannung leidet. Das ist hier nicht so! Es ist nicht leicht zu gewinnen, am ehesten noch, wenn wirklich alle voll konzentriert dabei sind, und oft genug steht das Spiel wirklich auf Messers Schneide. Durch die unterschiedlichen Rollen (und die Erweiterungen) muss man sich für jedes Spiel eine andere Strategie überlegen - und oft genug wird diese von der Pandemie über den Haufen geworfen. Wie im echten Leben. In Wirklichkeit, so kommt es mir vor, ist es aber ein Kommunikationspiel, das am Ende auch uns als Familie zugute kommt. (Unsere Chancen sind deutlich besser, wenn wir dem Jüngsten, eher ruhigen, zuhören, wenn er einen Vorschlag macht!)
Jetzt zu den Erweiterungen: Wir hatten die 2. Erweiterung (Ausnahmezustand) gleich dazubestellt und ein paar Wochen später ins Spiel gebracht. Das hat den Spielspaß nochmal deutlich erweitert und funktioniert auch ohne die 1. Erweiterung gut. Der Wunsch nach der 1. Erweiterung (Auf Messers Schneide / On the Brink) wurde dennoch größer, die aber anscheinend auf Deutsch nicht verfügbar ist. Für uns wäre es kein Problem, einen Teil des Spieles auf Englisch zu haben, sofern er mit der deutschen Ausgabe kompatibel ist. Aus den verschiedenen Rezensionen ließ sich das nicht genau ableiten. Wir sind dennoch das Risiko eingegangen und haben sie bestellt.
Fazit: Wir hatten das Grundspiel (Z-Man-Ausgabe, im Nov. 2020 bestellt) vorliegen und jetzt die englische Ausgabe "On the Brink" / "Auf Messers Schneide" dazubestellt - und sind rundum zufrieden. Die Karten passen sowohl vom Format als auch von den Rückseiten gut zusammen. Es scheint einen minimalen Unterschied in den Farbabstufung zu gegen, den man aber nur mit Mühe erkennt, wenn man zwei Karten direkt nebeneinander legt.
Bei den Rollenkarten gibt es einen deutlichen Unterschied, weil auf den englischen Rückseiten fett "Role" aufgedruckt ist. Das stört aber nicht, denn die Rollenkarten kann man leicht ohne hinzuschauen mischen und austeilen. Danach spielt es ja keine Rolle mehr.
Die fünf Petrischalen passen perfekt zu den Seuchenwürfeln unseres Grundspiels und werten das Spiel deutlich auf. Man hat das dringende Bedürfnis, sie nicht unnötig offen herumstehen zu lassen. :-) (Wer das Spiel nicht kennt: Das ist reine Deko und hat keinen Einfluss auf den Spielablauf.)
Dass die Ereigniskarten wie auch die Regeln und zusätzlichen Rollen nur auf Englisch vorliegen, ist auch deshalb kein Problem, weil man ja im Spiel nichts vor den Mitspielern verheimlichen muss. Man kann sich also Hilfe bei der Übersetzung holen oder sie gemeinsam besprechen. Notfalls reicht es, wenn einer aus der Runde die Sprache halbwegs ordentlich versteht.
Die Erweiterung macht großen Spaß, ist aber aus meiner Sicht nicht besser oder schlechter als die zweite, sie erhöhen jede für sich den Spielspaß und die Vielfalt deutlich. Wenn ich mich für eine entscheiden müsste, würde ich die zweite (Ausnahmezustand) vorziehen - weil die Ergänzung der "Quarantänen" das Spiel deutlich erleichtert und es dadurch mehr Spaß macht, die ganzen verschiedenen zusätzlichen Herausforderungen anzugehen. Das dürfte aber Geschmackssache sein.
Eine Anmerkung noch zur Spielerzahl: Wir haben es jetzt von 2 bis 5 Spielern gespielt, meistens zu viert. Der Spielspaß ist davon unabhängig, aber der Charakter ändert sich deutlich, weil immer unterschiedliche Strategien erforderlich sind.
